- During a recent audit of inmate classification files, it was determined that 6 inmates did not receive their required 30-90 day classification reviews. The delays ranged from 1 to 39 days, and all affected inmates had been in custody for 90 days or more, violating minimum standards
- During the review of restraint logs it was determined staff exceeded the 15 minute face to face observation required by minimum standards, by 1 to 6 minutes on 2 occasions. It was noted staff only utilized the restraint chair on 4 occasions during the last 12 months. Staff were reminded a documented observation of the inmate shall be conducted every 15 minutes, at a minimum. The observations should include an assessment of the security of the restraints and the circulation to the extremities.
