Notes and Summary
  • During the walk-through, this inspector observed that the fire panel was showing trouble and was unable to reset to normal mode. The panel is showing the sprinkler tamper switch is at fault. The vendor has ordered the part and is due to repair the panel on 9/15/17.
  • During the special inspection, this inspector observed that the inmates from McLennan (P) (Jack Harwell Detention Center) and Johnson (P) were not classified by the Limestone Detention staff prior to being housed. The staff from the LCDC were utilizing the classification forms from McLennan (P) and Johnson (P).
  • During the review of the inmate paperwork, this inspector observed that the approved screening instrument was not completed on the inmates from McLennan (P) and Johnson (P) when admitted to the Limestone Detention Center. The administration stated they were not aware that the screening needed to be completed again. It was the
    understanding of the administration that the previous completed screening at Johnson (P) and McLennan (P) were sufficient.
  • During the review of the activity logs, this inspector identified that the facility exceeded 14 hours between meals. On several occasions, namely Sept. 8th, Sept. 9, and Sept. 10, the facility was found to exceed 14 hours between meals between the dinner meal and the breakfast meal served the next day.
