Notes and Summary
  • Facility staff were unable to properly respond to a conducted fire drill. Staff were unable to utilize the facility SCBA and advised that they have received training in the use of the SCBA.
  • During the review of records, it was learned that staff have not verified the veteran status of each prisoner by using data made available from the Veterans Reentry Search Service (VRSS) operated by the United States Department of Veteran Affairs or similar service since November 2024.
  • The annual inspection of the facility by health authorities was last conducted January 18, 2024. This places the annual inspection past due by approximately two months.
  • At the time of the inspection, only one staff member possessed a valid food handlers license. Additionally, the staff member with a valid food handlers license was not present in the facility on the day of the inspection.
  • Recreation documentation reviewed for the month of January 2025 failed to show that inmates are allowed one hour of supervised physical exercise or physical recreation at least three days per week. Additional documentation reviewed for the months of December 2024, February 2025 and March 2025 also failed to show that inmates are allowed one hour of supervised physical exercise or physical recreation at least three days per week.
