- During the review of 8 restraint logs, consisting of 76 total observation checks, 14 of 76 checks were found to have exceeded the required 15-minute observation period by 1 to 16 minutes.
- Staff failed to provide at least twenty-four hours written notice to the inmate of the claimed violation or charges against him/her before proceeding to offer a waiver or holding a hearing on 4 out of 20 inmate disciplinaries reviewed. This was an area of technical assistance on the Limited Compliance Review conducted on January 10, 2025.
- Recreation logs reviewed for the month of September 2025 failed to show that inmates are allowed one hour of supervised physical exercise or physical recreation at least three days per week. Additional recreation logs reviewed for August 2025, and October 2025 also failed to show that inmates allowed one hour of supervised physical exercise or physical recreation at least three days per week.
