On three occasions the date and time of notification to the magistrate was absent, when notification was required. On one occasion the date and time of notification to the LMHA was absent, when notification was required. On one occasion, there were no comments made as required due to an affirmative answer to the screening questions. In addition, one screening form had no CCQ match indicated when a positive CCQ return was identified.
On one occasion sanctions were administered prior to a waiver or hearing being offered.
