• During the review of life safety documentation, it was determined the facility has not been inspected by a certified fire official since 10-25-2017 and is past due.
• During inspection of the fire extinguishers, it was determined that the extinguishers were last inspected in August 2018 which places the inspection three months past due.
• During the walk-through, this inspector determined that the facility roof is leaking and is in need of repair.
