On the day of the inspection, it was observed that the last fire marshal inspection was performed on 2/8/2018 which is over one month past due.
During the fire drill, it was determined that the smoke evacuation system did not engage automatically when the artificial smoke was introduced to the smoke detector head as required.
No documentation could be provided by administration that jail staff are receiving the 60 minutes of suicide prevention training annually in accordance with the agency’s approved operation plan.
During a review of observation logs, it was determined that jail staff are exceeding the required face-to-face 60 minute observation on a continual basis by as few as 1 minute
up to 200 minutes.
During a review of recreation logs, it was determined that inmates are not being offered recreation three times in a seven day period. The one hour timeframe is also being shortened with no documented explanation.
