- During the walkthrough, it was determined that the smoke removal system in P Dorm and the Laundry Room were not automatically engaging as designed when alerted by the fire control panel.
- During the walkthrough, it was observed that two fire panels (M Dorm and Visitation) and a few fire extinguishers were red tagged throughout the facility.
- It was determined that inmates housed in single cells for administrative separation are not always being provided access to a day room for at least one hour each day on the days that recreation is being offered.
- During a review of restraint logs, it was determined that documented 15 minute observations were exceeded from 1 minute up to 105 minutes.
- During a review of inmate files, it was determined that jail staff are not obtaining a written acknowledgment that the rules have been explained on any of the current files.
