Notes and Summary

 

• The facility was inspected on September 8, 2018 but 38 violations of NFPA exist. As of the date of inspection the facility has not been re-inspected and remains noncompliant.
• The facility’s TB plan was last approved on January 2018 which places it three months overdue.

Very disturbing violations that were found back on September 8th 2018 and have not been remedied. TB plan is out of date and 38 NFPA violations give us great cause for concern.
