- After reviewing submitted documentation and video evidence in conjunction with selfreporting by jail administration, it was determined that face-to-face observations were either conducted late or not conducted for 1 hour up to 3 hours on 30-minute and 60-
minute face-to-face cell observations during the day shift on January 20, 2020, January 28, 2020 ,February 1, 2020 and February 2, 2020.
This is one of the most common and life threatening violations. Mandatory face to face observations every 30 minutes of persons with mental illness on suicide watch.
