Notes and Summary

• Quarterly fire safety training was not completed for 1 jailer, and 4 other jailers had not received training during one or two quarters of the year
• Generator testing was not documented during the majority of the year. All fire extinguisher inspections expired November 30, 2021
• Staff exceeded the 30 minute face to face observation checks from anywhere between 1 and 10 minutes in holding and detox cells
• Staff exceeded the 60 minute face to face observations from anywhere between 1 and 12 minutes
• Kitchen area was found to be dirty without food observed to be dried to appliances, as well as around the floor and the food tray carrier/cart. In the laundry, the dryer was found to have a large amount of lint in the trap. Cell. #104 was found to have a large amount of black substance (mold) on the shower walls
• Multiple plumbing issues: low water pressure, no hot water, violations of American Disabilities Act
