- While reviewing the restraint chair usages this inspector learned that there was no log being utilized to document these instances
- While reviewing the air pack/ fire drill training the inspector found on the training documents for this area of training that on multiple entries the dates did not coincide with the quarter listed on the training document. Additionally, on multiple occasions/ quarters multiple staff members did not receive this training as the minimum standard dictates.
