• 5 of 32 checks conducted [in a holding cell or detoxification cell] were in excess of the 30 minute requirement from 5 to 37 minutes.
• 55 of 466 observation checks conducted were in excess of the 60 minute requirement from 10 minutes to 3 hours and 16 minutes.
• Out of 74 instances inmates should have been offered one hour of supervised physical activity or physical recreation at least three days per week, 21 times no offer was made.
• No load test was conducted for the month of September 2025. In addition, documentation provided revealed that no weekly run test was conducted for the following 10 weeks: September 21-27, 2025; September 28-October 4, 2025; October 5-11, 2025; October 12- 18, 2025; October 26-November 1, 2025; November 16- 22, 2025; December 7-13, 2025; December 14-20, 2025; May 3-9, 2026; May 10-16, 2026.
