During a review of 5 suicide/mental health screening forms it was discovered that on 3 occasions the supervisor was not notified as required by responses provided by the inmate. Information provided on one screening form indicated the magistrate must be notified. Review of the magistrate notification form indicated no notification was required, contradictory to information the screening form, resulting in a failure to notify the magistrate as required by CCP 16.22. Further, on 2 occasions, the magistrate notification time entered on the screening form does not match the time entered on the magistrate notification form, differing by as much as 10 hours.
Administration was unable to provide any logs to indicate inmates were receiving 3 days of recreation as required.
