An inmate was in the holding cell for 8 days and was not given the bedding required by TCJS 277.8.
On 8 of the 9 files reviewed, the property inventory was not signed by the inmate and no notice of refusal to sign was indicated.
On one occasion staff failed to identify a detainer on the initial classification form. On two occasions staff failed to identify an assaultive felony on the initial classification form.
A classification audit had not been conducted since May 2025, in contradiction with the Dimmit County Operational Plan.
On 3 of the 9 files reviewed, proper notification was not made to the magistrate or LMHA, in accordance with CCP 16.22, which requires intake screenings to identify inmates who are known to be or observed to be mentally disabled and/or potentially suicidal and make referrals to available mental health officials
On 8 of the 9 files reviewed there was no written acknowledgment by the inmate that the rules of inmate conduct have been explained.
