  • During a walkthrough of the intake holdover housing area, documented observation logs of suicidal people were found to be incorrectly documented when compared to video documentation
  • Documentation was not available to show that people considered as "crisis" were provided a change of clothing at least once a week
  • Documentation was not available to show that washable items such as sheets, towels, and mattress covers were provided to people considered as "crisis"
  • During a walkthrough of Dallas County Jail's North Tower, approximately 40 multiple occupancy cage doors were identified to be in disrepair compromising the "safety" and "security" of the jail
