Notes and Summary
  • Jail staff on multiple occasions failed to notify the magistrate within 12 hours (in accordance with CCP 16.22) when warranted by positive returns on the Continuity of Care Query or answers on the Screening Form for Suicide and Medical/Mental/Developmental Impairments.
  • During review of medical files, it was determined staff failed to complete the Screening Form for Suicide and Medical/Mental/Developmental Impairments in its entirety.
  • Observation checks of people restrained with the restraint chair were conducted from 2 to 17 minutes over the required 15 minute time frame on multiple occasions.
  • Multiple access panels were missing screws or had loose screws. Security screws are required to prevent tampering.
  • Camp County Jail switched to an annual audit of the classification system without submitting changes to the operational plan to TCJS.
  • Multiple mistakes were observed on classification assessments and reassessments.
  • Technical assistance provided by TCJS on all areas of deficiency.
