- While reviewing 10 medical files 4 errors were noted. It was determined that jail staff are not filling out the Screening Form for Suicide and Medical/Mental/Developmental Impairments in its entirety.
- A review of ten inmate medical files revealed that jail staff on two occasions failed to notify the magistrate within 12 hrs. in accordance with Code of Criminal Procedure Article 16.22 when warranted by positive returns on the CCQ or affirmative answers on the Screening Form for Suicide and Medical/Mental/Developmental Impairments.
- A review of staff licensing and staff interviews revealed that four staff members licensed only as a Telecommunications Operator are performing jailer duties without possessing a valid jailers license.
- During the review of staff licensing, it was determined that two staff members appointed as temporary jailers were not registered for a basic licensing course on or before the 90th day after appointment.
