Notes and Summary

• Observation shall be performed at least every 30 minutes in areas where inmates known to be assaultive, potentially suicidal, mentally ill, or who have demonstrated bizarre behavior are confined.   A review of video submitted after an escape revealed that while rounds were conducted as mandated by minimum jail standards, face-to-face observations were not conducted. The escape occurred at approximately 0300 hours, and it was not discovered until approximately 0915 hours that the inmates were missing.
