- A review of inmate medical files determined staff failed to notify the magistrate as required by CCP 16.22 and the Screening Form for Suicide and Medical/Mental/ Developmental Impairments. Further review revealed that jailers are not completing the form in its entirety by not documenting required comments when answering “Yes” to
questions 1-16, notating CCQ matches, and filling out the required notification dates and times at the bottom of the form.
