The magistrate was being notified; however, the magistrate notification form was filled out incorrectly and did not advise the magistrate of the indicators documented on the mental health screening form. Also, the staff are not sending a copy of the screening form to the magistrate nor was the CCQ match reported as required by Article 16.22 of the CCP and minimum jail standards.
The Screening Form for Suicide and Medical/Mental/Developmental Impairments was not being filled out, in its entirety, as required by minimum jail standards.
The last health inspection occurred on January 22, 2018. This places the inspection past due by 9 months.
