Notes and Summary
  • Documented 15 minute observations of an inmate restrained in the restraint chair were not conducted.
  • Observations on inmates in areas where inmates known to be assaultive, potentially suicidal, mentally ill, or who have demonstrated bizarre behavior are confined routinely exceeded the required 30 minutes time frame.
  • A review of suicide training revealed that multiple staff members did not complete a least 4 hours of suicide training in accordance with the approved operational plan .
  • Jail staff on multiple occasions failed to notify the magistrate within 12 hrs. in accordance with Code of Criminal Procedure Article 16.22 when warranted by positive returns on the CCQ or affirmative answers on the Screening Form for Suicide and Medical/Mental/Developmental Impairments .
  • Records reviewed shows that multiple staff members did not receive required life safety training during the fourth quarter of 2022
  • The facility was last inspected January 22, 2022, placing the annual inspection overdue by 1 month.
  • Observations of inmates in holding and detoxification cell routinely exceeded the required 30 minute period .
  • A log of positive VRSS returns with identifying prisoner number and whether a referral card was issued to the identified veteran prior to his or her release has not been maintained for the preceding two months .
  • While reviewing medical files, it was determined that jail staff are not filling out the Screening Form for Suicide and Medical/Mental/Developmental Impairments in its entirety.
