Notes and Summary

• Staff had not been received Suicide Prevention Training during 2021 as required by the Fisher County Mental Disabilities/Suicide Prevention Operational Plan. This was a deficiency during 2019 annual inspection.
• On more than one occasion, magistrates were not notified of 16.22 screening forms for Suicide and Medical/Mental/Developmental Impairments. This was a deficiency during 2019 annual inspection.
• It was determined that the fire marshal conducting safety inspections is not certified as a fire inspector.
